Our Billing Policy and Insurance Information

Apple Hill is committed to providing you with high quality eye care services and exceeding your expectations. We continually strive to find ways to improve the services we provide to you, our patient. We know dealing with insurance companies can be confusing and time-consuming, but we’re here to help. Patients are responsible for payment of charges. Our business office will file your charges with medical health plans in a timely manner. While we do not submit claims to Vision plans, we will provide you with the information you need to submit a claim. You are responsible for any co-pay, co-insurances, deductibles, as well as refraction fees not covered by insurance. We require that these be paid at the time of service. Any amounts not covered by insurance will be billed to the patient. Because your insurance is an agreement between you and your insurance company, we do not become involved in disputes between you and your insurance company regarding deductibles, co-payments, non-covered or denied services. If you have any questions or concerns regarding the payment of a bill, please call a billing representative at 741-6732.

We will send you a monthly statement of any unpaid charges, along with a return payment envelope for your convenience. You may pay by cash, check, MasterCard, Visa, Care Credit or Discover. If you are unable to pay the balance due within 30 days, please contact our Billing Department to set up a payment plan.

In order to expedite the processing of insurance claims and payments, we need your assistance. Please:

  • Verify that AHEC is a contracted provider for your specific insurance plan
  • Present your insurance card(s) at each visit.
  • Promptly report any changes to your insurance plan.
  • Obtain any necessary referrals from your primary care physician prior to the visit.
  • Be prepared to pay any co-payments, deductibles, and non-covered services performed during your visit. Co-payments are usually noted on your insurance card. Refractions are not covered by Medicare and many other health insurance plans.
  • If you are a member of an insurance plan with which we do not participate, we will file your claim as a courtesy for you; however, you are responsible for paying for your services at the time of your visit.
  • Once your claim has been processed, any reimbursements due you will be paid by refund check within three weeks of the overpayment.

If you need to cancel an appointment, a 24-hour advance notice is required.
